The Case Manager is responsible for developing, implementing, and monitoring the program planning process for assigned caseloads of individuals. This role involves owning the case and all related paperwork from pre-admission to discharge, ensuring compliance with regulatory requirements and internal guidelines. The Case Manager will also manage funding, review and maintain goals/outcomes, chair meetings, build rapport with individuals, and collaborate with various departments. Fluency in computer use for documentation and reporting is essential, as is providing support to individuals and staff. The position requires completing training, maintaining organized files, evaluating skills to enhance programming, and serving as a liaison between programs, families, and agencies. The role also involves working varied hours, including evenings and weekends, and participating in committees and special projects. Cooperation and cross-training with other case managers are also expected.
