About The Position

YouthLink is seeking a Chemical Health Case Manager to join their team. This role involves providing comprehensive case management services to individuals and families affected by chemical health issues, such as addiction and substance use disorder. The Case Manager will develop individualized care plans, coordinate services with other providers, assess client needs, advocate for clients, and provide education and resources to support their recovery. This position is ideal for someone passionate about chemical health and helping those in need.

Requirements

  • 1 year of case management experience
  • 2 years’ experience working with clients experiencing homelessness
  • 2 years’ experience with community engagement
  • 2 years’ experience working with clients going through chemical health issues
  • Must be able to come to work on time, for all regularly scheduled work shifts
  • Valid MN State Driver’s License and insurance

Nice To Haves

  • Knowledge of chemical health, substance use disorders, addiction services, recovery principles, and harm reduction practices.
  • Ability to assess client needs, develop individualized treatment plans, and provide effective case management services.
  • Knowledge of community resources and the ability to coordinate care, provide referrals, and advocate for clients.
  • Ability to provide crisis intervention, de-escalation, problem-solving, and support during challenging situations.
  • Strong understanding of trauma-informed, youth-centered, and culturally responsive service delivery.
  • Excellent interpersonal, verbal, and written communication skills with the ability to engage diverse populations.
  • Strong organizational, time management, and multitasking skills with the ability to manage competing priorities.
  • Ability to work independently and collaboratively within a multidisciplinary team environment.

  • Ability to build and maintain professional relationships and boundaries with youth, co-workers, partners, and community stakeholders.

  • Ability to positively represent the agency and uphold YouthLink's mission, values, and guiding principles in all interactions.
  • Passion for supporting youth and young adults and helping individuals achieve stability, recovery, and self-sufficiency.
