About The Position

The Child and Family Guidance Center is seeking a Bi-Lingual Part-Time Care Coordination (Case Manager). This is a part-time 17.5-hour position that may become full-time in the fall. The Bi-Lingual Part-Time Care Coordinator is responsible for providing care to children with complex emotional and/or behavioral health challenges using the Wraparound process. This position requires a bachelor’s degree in human services, fluency in Spanish and English, a valid driver’s license and auto transportation to external locations.

Requirements

  • A bachelor’s degree in social work, sociology, psychology, or in the human service field
  • Fluency in Spanish and English
  • A valid driver’s license with auto transportation and ability to travel to and from client family’s home on a regular basis

Nice To Haves

  • Experience providing direct services as a case manager to families with complex behavioral health needs is preferred.
  • Cultural sensitivity, awareness and knowledge required as well as a strong familiarity of community resources preferred
  • Bi-lingual Spanish speaker is preferred.

Responsibilities

  • Provide Care Coordination Case Manager services to 10-12 children/youth with complex emotional and/or behavioral health needs.
  • Maintains fidelity to the Wraparound process when working with all assigned families.
  • Develops a crisis plan for each family served that guides the family to appropriate assistance, supports and services in the event of a family mental health crisis.
  • Assists the family with developing and convening monthly Child and Family Team Meetings (CFT) of traditional and non-traditional providers with the purpose of developing a Plan of Care (POC) that is culturally appropriate.
  • Ensures that plans of care and crisis plans are implemented appropriately and adapted by the CFT as needed to meet the multiple and changing needs of the child and family.
  • Provides weekly contact with families served and maintains strength-based, outcome focused documentation of all contacts.
  • Adhere to CFGC guiding principles.
  • Maintains a Wraparound Family Record (WFR) on all children and families receiving Care Coordination Services.
  • Successfully transitions families out of Care Coordination services by building natural supports, sustainable strategies and pro-active crisis planning.
  • Gather and submit data required by DCF in a timely manner.
  • Assists in program evaluations such as outcome measures and client satisfaction surveys to families and youth (children as appropriate) during service delivery and upon discharge from Care Coordination Service.
  • Works in conjunction with community providers such as DCF, Probation, Schools, etc., to ensure seamless, collaborative care coordination for children/youth.
  • Assists in identifying service gaps and in locating resources within the community.
