About The Position

Case Management is a vital part of the H-NNCSB service delivery system, serving as a unifying link between families, children, and the supports they need. The Intensive Care Coordination (ICC) Case Manager offers a higher level of support, including clinical assessments and level-of-care determinations, for individuals diagnosed with a Serious Emotional Disturbance (SED) or an Intellectual or Developmental Disability (IDD). This role exists to ensure services are timely, appropriate, and aligned across schools, providers, and community supports.

Requirements

  • Bachelor’s degree in a Human Services field such as Social Work, Psychology, or Counseling
  • Two (2) years of experience providing mental health case management or care coordination services for children
  • Knowledge of DSM-5 diagnosis and care management principles

Responsibilities

  • Completing clinical and service assessments
  • Developing individualized service plans
  • Coordinating referrals and authorizations
  • Tracking progress toward goals
  • Maintaining accurate documentation
  • Providing advocacy and crisis response as needed in partnership with the multidisciplinary team
