About The Position
Case Management is a vital part of the H-NNCSB service delivery system, serving as a unifying link between families, children, and the supports they need. The Intensive Care Coordination (ICC) Case Manager offers a higher level of support, including clinical assessments and level-of-care determinations, for individuals diagnosed with a Serious Emotional Disturbance (SED) or an Intellectual or Developmental Disability (IDD). This role exists to ensure services are timely, appropriate, and aligned across schools, providers, and community supports.
Requirements
- Bachelor’s degree in a Human Services field such as Social Work, Psychology, or Counseling
- Two (2) years of experience providing mental health case management or care coordination services for children
- Knowledge of DSM-5 diagnosis and care management principles
Responsibilities
- Completing clinical and service assessments
- Developing individualized service plans
- Coordinating referrals and authorizations
- Tracking progress toward goals
- Maintaining accurate documentation
- Providing advocacy and crisis response as needed in partnership with the multidisciplinary team
