About The Position

CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports, they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. Scattered Site Housing Program provides 270 scattered site-housing units to HIV/AIDS Services Administration (HASA) clients. All the units are leased in the agency’s name. Sixty-seven units are dedicated for families and 203 units are for single individuals. The program has two components to assist clients succeed in housing, socials services and operations. Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy.

Requirements

  • Bachelor’s Degree OR Associates degree/high school diploma/GED with at least 4 years’ experience in case management or related social service field (housing, homelessness, mental health, substance abuse, etc.).
  • Two (2) years of relevant experience, preferably in a mid-sized (250+ employees) organization working with persons with a histories of homelessness, mental illness, and/or substance abuse.
  • Good written & verbal communication.
  • Computer literacy in Microsoft Office Suite.
  • Bi-lingual English and Spanish.

Responsibilities

  • Providing all contract funded activities, including conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, etc.
  • Completing all program documentation and entering all client services into Client Track.
  • Ensuring clients are connected to and maintain primary medical care and adhere to medical and medication treatment.
  • Connecting clients to needed services, such as, medical, mental health, substance use treatment, as well as, legal, entitlements, etc.
  • Monitoring clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and documenting via progress notes.
  • Functioning as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.
  • Escorting clients to appointments (educational, medical, social service, etc.).
  • Assisting clients in completing applications for benefits and entitlements and monitoring outcome of process.
  • Collecting all required documentation and creating client files.
  • Conducting a minimum one home visit per month.
  • Reporting any repair issues to the Housing Specialist.
  • Monitoring clients’ living conditions.
  • Meeting required monthly projections.
  • Providing all required information for the completion of monthly program reports.
  • Maintaining clients’ charts always audit ready.
