About The Position

Clinica Sierra Vista is seeking a dedicated Case Manager-BH to join their team. This role involves coordinating resources and services for clients, providing case management, and assisting mental health clinicians. The position emphasizes an interdisciplinary team approach to comprehensive client care and advocates for clients' rights to mental health information and participation in their care planning. The ideal candidate will have a strong belief in providing mental health care with dignity for all.

Requirements

  • Completion of a Bachelor’s degree from an accredited college, or university, with a major in Psychology, Sociology, Human Services, Behavioral Science, Social Work or related field.
  • Culturally competent and demonstrate ability to engage with patients of the multi-cultural backgrounds, nationalities, origins and diverse sexual preferences.
  • A clean drug screen confirmation.
  • Pass DMV background check.

Responsibilities

  • Coordinating resources and services for clients.
  • Providing case management services.
  • Assisting the mental health clinician in the development and delivery of services to mentally ill individuals.
  • Participating in group and individual counseling programs.
  • Acting as an advocate for the client.
  • Creating and maintaining relationships with community groups.
  • Completing documentation in case records.
  • Assisting the client to complete appropriate releases of information important to client compliance with individual plans of care.
  • Advocating for clients when there is a problem in the service delivery system.
  • Assisting clients in identifying and correcting situations that contribute to mental health problems.
  • Performing crisis intervention counseling at a level not requiring licensure.
  • Assisting clinicians in planning the range of care needed to meet clients’ needs.
  • Maintaining assigned caseload and client contacts as required by contract requirements and/or program protocols.
  • Visiting clients regularly in their homes and in the community to assess their home situations, deliver services, and determine if other services are required.
  • Keeping accurate, up-to-date records on clients served in accordance with system standards.
  • Preparing and delivering oral presentations to the public regarding Clinica Sierra Vista’s mental health services program.
  • Working with other staff to develop community resources.
  • Serving as liaison with other community agencies and schools.
  • Developing and implementing support and educational groups.
  • Being available to translate for specific sessions, if qualified.
