Job Overview The Case Manager of Behavioral Health is responsible for providing referral management for the Substance Abuse and Behavioral Health patient population. The Case Manager follows the program’s Scope of Work to achieve the program goals through the screening of patients, referral organization, referral management, appropriate linkage of care, and record keeping.

Requirements

Bachelor’s degree in Social Services, Sociology, Human Services, Counseling, or a minimum of 5 years working in health care assisting patients with case and referral management and linkage to care.
Bilingual English/Spanish or English and other predominant language of the population served preferred.
