About The Position

In this fast-paced environment, The Behavioral Health Case Manager will provide comprehensive case management services to individuals with mental, emotional, or substance abuse disorders in an outpatient setting. The position will coordinate care, track progress and ensure clients receive access to necessary services and resources to support their overall health and well-being. The Case Manger will develop and implement treatment plans and monitor progress in collaboration with healthcare providers, community organizations and other support systems to ensure clients receive holistic, patient centered care. The position will be required to provide ongoing support, advocacy and education and serve as a point of contact for clients and their families.

Requirements

  • Bachelor’s degree in social work, psychology, sociology, healthcare or medical and related fields
  • Current CPR/BLS certificate.
  • Possesses superb written and spoken communication skills.
  • Excellent interpersonal skills with colleagues, community leaders, policymakers and others.
  • Excellent time management skills; organized and able to prioritize.
  • Motivated to take on additional community involvement projects and solve problems.
  • Comfortable in a fast-paced environment with multiple cases.
  • Able to organize and manage large amounts of files, schedules, dates, and information.
  • Self-directed and able to work without supervision.
  • Comfortable with building personal relationships and dedicated to helping others.
  • Empathetic and supportive with mentorship and leadership skills.
  • Licensed or certified by state board according to regulatory.
  • Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel).
  • Able to work nights and weekends.
  • Able to travel to meet with clients.

Nice To Haves

  • Community Health Worker Certificate a plus
  • Master’s degree in social work preferred
  • supervised practicum or internship preferred

Responsibilities

  • Assess clients to determine needs in the areas of behavioral health, mental health, substance use or social services and develop individualized treatment plans to address short and long term goals.
  • Coordinate, facilitate and advocate for clients to ensure they access required services and resources such as behavioral health services, medical care, housing employment and other community resources.
  • Establish good working relationships and exert influence with support networks, government resources, and community resources to assist and provide support and resources for clients.
  • Organize and influence community and other agencies to develop programs, policies, and services to assist in social work.
  • Serve as the primary point of contact for clients and their families and ensure that services are timely, appropriate, and aligned with patient-centered treatment plans.
  • Help manage crises by developing crisis management plans, providing support during crisis and helping clients develop coping mechanisms.
  • Serve as a key member of an interdisciplinary team of physicians, nurses, social workers, therapists, and other providers, to ensure a coordinated approach to care.
  • Participate in case conferences and share relevant information to ensure effective client care and outcomes.
  • Create programs and services to benefit clients and community.
  • Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and follow-up reports, in compliance with agency policies and regulatory requirements.
  • Ensure documentation is completed timely and in accordance with confidentially rules and HIPAA compliance.
  • Assist with any other duties as assigned.
