About The Position

The Case Manager collaborates with the multidisciplinary team to identify, assess, plan, implement, coordinate, monitor, and evaluate options and services in meeting the needs of our patients. The Case Manager coordinates the care of the patient while in the hospital to develop safe discharge plans which reduce inpatient length of stay and hospital readmissions.

Requirements

  • Bachelor's Degree in health care related field
  • BLS (American Heart Association)
  • CPI certification within 90 days of hire
  • Fingerprint Clearance Card application number upon hire
  • Fingerprint Clearance Card within 90 days of hire
  • Colleagues must have an understanding of computers and competence in using computers and basic software programs.

Nice To Haves

  • Master's Degree in a related healthcare field from an accredited college or university
  • 2 years of experience in healthcare

Responsibilities

  • Meets with patient to identify any potential barriers to discharge and presents information to MDT.
  • Coordinates care conferences with outside agencies.
  • Obtains/processes necessary documents to initiate and complete placement and other arrangements.
  • Arranges/maintains appropriate transportation requests as needed.
  • Schedules follow-up appointments and document in EMR of patients.
  • Interfaces with Utilization Review case managers to stay current on patients eligibility for admission, continuing stay, and readiness for discharge.
  • Works with insurance companies and service units to relay necessary information for service authorization.
  • Collaborates with Central Business Office to track payer denials and develop appeal documents.
  • Procures patient's medications and/or DME prior to discharge.
  • Understands basic medical terminology and DSM-5 diagnoses.
  • Communicates with the team to keep them informed of the progress of their request.
  • Collaborates and provides useful resources regarding community services to the patients, as well as offering information on the patient assistance fund with appropriate discretion to support patient needs.
  • Works in collaboration with the physicians, outside offices, and hospital staff on the status of the patient.
  • Participates in creating and sustaining a learning environment supported by evidenced-based practice and research.
  • Focuses on personal development areas, engaging in professional/collegial interactions with peers and students.
  • Works collaboratively with peers and other healthcare providers and participates in ongoing in-service and educational seminars.
  • Completes all documentation accurately and in a timely manner.
  • Learns to/administer TMS (Transcranial Magnetic Stimulation) treatments and supports the patient during treatments using positive communication and reinforcement to ensure optimal results are achieved.
  • Supports patient education activities as directed by the Clinical Manager
  • Responsible for reporting any safety-related incident in a timely fashion through the Midas/RDE tool; attends all safety-related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
  • Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.
  • If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
  • Completes all company mandatory modules and required job-specific training in the specified time frame.
