As a team member for ND Department of Health & Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover. As a member of the youth and family team, you will provide youth and families with opportunities to manage their recovery so they can live and work independently in their community. In this unique role, as a Case Manager & Skills trainer, with the support of an interdisciplinary team, you will conduct comprehensive needs assessments, complete and implement service plans, as well as leverage contacts and referral partners to ensure access to needed services. Every day, you will travel to individuals' homes and communities. There, you will teach daily living and household skills, communication, and emotion regulation skills, budgeting skills, as well as health, safety, and hygiene skills to children and their caregivers. You will be part of an integrated, interdisciplinary team focused on providing the best support plan possible for everyone. Case Management/Skills services are community-based. Work will be done primarily in the individual's home and community between the hours of 10:00 am and 7:00 pm.
