Our Youth Case Manager meets with the youth and family of minors placed in care by DCFS due to abuse or neglect by caretakers, to create a treatment plan with the ultimate goal of permanency. This role helps identify their individual goals and service needs, coordinates wrap around services in the community, and assesses the client and family progress. This role meets with youth clients 1-3 times a month, family members once a month, and will observe family visitation once a month. You may provide transportation for visits, doctor appointments, or other meetings with the client, legal team, DCFS team, or family. You will be provided regular supportive supervision by the Child Welfare Supervisor to aid in your own professional growth and development.
