About The Position

The Reentry & Recovery Case Manager Assistant supports individuals transitioning from incarceration, treatment programs, or unstable living situations into stable, independent lives. This role works closely with R&R Case Manager(s) to coordinate services, track progress, and provide hands-on support to clients navigating recovery, housing, employment, and community reintegration. This position plays a critical role in reducing recidivism, supporting long-term recovery, and helping individuals build stable, productive lives within their communities.

Requirements

  • Excellent communication and interpersonal abilities
  • Ability to maintain professional boundaries and handle sensitive situations
  • Basic computer skills (case management systems, Microsoft Office, etc.)
  • Trauma-informed and culturally responsive approach
  • Ability to de-escalate challenging situations
  • High school diploma or equivalent required
  • Must be 21 years of age with a valid driver’s license
  • Experience working with justice-involved individuals, people in recovery, or vulnerable populations

Nice To Haves

  • Lived experience with recovery or reentry (valued but not required)
  • Peer support certification
  • Associate degree in social work, psychology, criminal justice, or related field preferred

Responsibilities

  • Assist case managers in developing and implementing individualized case plans
  • Help coordinate access to housing, healthcare, mental health services, substance use treatment, tribal resources, employment services, and other community resources as indicated in case plan.
  • Maintain accurate, up-to-date client records and documentation in the Electronic Health Records System
  • At direction of Case Managers, conduct client check-ins to monitor progress and identify needs
  • Provide transportation assistance or accompany clients to appointments when needed
  • Support clients in completing applications (housing, jobs, benefits, IDs, etc.)
  • Facilitate or assist with group sessions, workshops, or life skills training
  • Track and document client outcomes, including at time of discharge and six months after discharge
  • Build relationships with community partners and service providers
  • Ensure confidentiality and compliance with organizational policies and regulations
