About The Position

This role provides engagement and case management support services to individuals in the program. The Case Manager Aide will assist the program manager and case managers, ensuring services are delivered in compliance with contract and Agency regulations. The position involves outreach activities to engage participants, providing clinical services as needed in natural environments or program housing, and assisting participants in accessing mainstream clinical services. A key aspect of the role is working with participants to develop, implement, and monitor personal goal plans to help them achieve housing stability.

Requirements

High School diploma or General Equivalency Degree (GED) and at least 2 years of direct care experience working with the target population; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
Basic understanding of Mental Health, and/or Substance Abuse issues.
Very good communication skills.
Detail oriented.
Ability to work independently and as part of a focused team.
Must be available for local travel and possess a valid Driver's license.
Possess manual dexterity and fine motor skills.
Will be required to carry a mobile phone during regularly scheduled working hours.
