About The Position

The Case Manager for Adult Mental Health Services provides case management and clinical services for individuals with mental illness, substance use disorders, and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.

Requirements

Bachelor's Degree in psychology, counseling, social work or special education.
Registration as a QMHP is required.
Must maintain QMHP status and all required trainings/CEUs.
At least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required.
