About The Position

The Youth and Family Services, Multnomah County Department of County Human Services (DCHS) invites applications to fill: One (1) Full Time Case Manager 2 located at the Gateway Center for Domestic and Sexual Violence, 10305 E Burnside St, Portland, OR 97216. This position is with a Hybrid schedule 40 hours/week Monday – Friday, 8:00 am. - 5:00 pm. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations. The eligible list created from this recruitment may be used to fill future regular, limited duration, full or part time, temporary, or on-call positions within DCHS. The Housing Assessor Case Manager 2 provides case management services to address the housing needs of survivors of domestic and sexual violence. The Housing Assessment Case Manager will assess housing and shelter needs and match survivors to a variety of available shelter and housing services under the supervision of the manager of the Gateway Center for Domestic and Sexual Violence Services.

Requirements

  • Any combination of experience and training that would likely provide the required knowledge and abilities is qualifying.
  • Equivalent to an Associate's degree from an accredited college or university with major course work in social science, behavioral science, or related field; AND One (1) to four (4) years of experience working with survivors of domestic and sexual violence.
  • Must pass a criminal background check (this step occurs after a job offer)
  • Must have a valid driver’s license

  • Understanding of housing resources in Multnomah County and experience assisting clients with housing solutions.
  • Preferred knowledge and expertise regarding service standards for clients experiencing domestic and sexual violence.

Responsibilities

  • Perform complex case management duties, including assessing/evaluating/documenting eligibility for housing services in accordance with local and programmatic standards and guidelines.
  • Conduct intakes, including Safety Stabilization Assessment, explanation of eligibility requirements with program participants, and answer questions they may have.
  • Conduct comprehensive needs assessment and a detailed and specific service plan with consumers to ensure an accurate understanding of how housing services will improve the consumer's safety and stability.
  • Develop detailed housing plans with consumers and ensure proper releases of information are in place to provide a wide range of liaison functions with landlords and other housing partners.
  • Ensure proper tracking of clients and assessments.
  • Ensure that elements of service plan are in place, considering, coordinating and/or arranging services as needed to fully address assessed needs.
  • Engage in problem resolution and/or facilitate other referrals to other agencies as needed.
  • Work with other agencies and professionals as indicated.
  • Represent Gateway Center in coordinated access system for domestic and sexual violence programs.
  • Submit client referrals for Resource Coordination Team (RCT) long-term housing considerations.
  • Participate in RCT and other meeting and other activities that support collaboration with the entire domestic and sexual violence continuum.
  • Have comprehensive knowledge of all available housing resources and ensure referrals to services meet all criteria.
  • Engage in housing stabilization activity for consumers who are receiving domestic and sexual violence advocacy services, including: supporting relocation to safe locations, or providing eviction prevention or rental assistance for their existing housing.
  • Request payments and track appropriate documentation regarding client assistance distribution.
  • Timely submit promissory notes to landlords.
  • Timely submit payment requests according to the DCHS AP standards.
  • Keep detailed records of all client assistance transactions.
  • Prepare and provide monthly reports to the Gateway Center management team with details on activities and results.
  • Document all direct and indirect contact with consumers.
  • Provide ongoing support and a point of contact for consumers related to their housing needs.
  • Follow up with consumers to assess outcomes and additional needs.
  • Perform other duties as assigned.
