Case Manager 2 - Cornerstone (New Haven)
About The Position
This position will be responsible for providing case management services to a specified caseload of residents, supportive counseling, psycho-social activities and coordinating resident services. The Case Manager 2 functions as a team member of the program and as a member of designated resident treatment team. This position is responsible for carrying a caseload, for implementing and facilitating resident activities/interventions, and for providing program shift coverage. Working within Adult Services, the Case Manager 2 works on the Behavioral Health Treatment care team to support individuals with severe and persistent mental illness residing in a residential setting.
Requirements
- High School Diploma or GED
- 3+ years of relevant experience
- Valid Driver’s License
- Regular and predictable attendance is required
- Newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
