About The Position

The Case Management Supervisor provides leadership and case management oversight within Mooseheart's residential facility for Intellectually and Developmentally delayed youth. This position ensures that services delivered to youth are person-centered, culturally responsive, and aligned with agency expectations and DCFS regulatory requirements. The Case Management Supervisor also provides formal supervision, guidance, and operational oversight to Child Care Supervisors to ensure consistent, high-quality daily operations within the home.

Requirements

  • Candidates must meet all regulatory criteria for Case Management Supervisors, including:
  • Minimum age of 25 years
  • Master's degree in social work or human/social services
  • At least 1 year of experience in child welfare or a human/social services setting.
  • Current Qualified Intellectual Disability Professional (QIDP) certification.

Responsibilities

  • Provide comprehensive case management using person-centered approaches that fully include youth in decision-making, daily routines, and goal planning while promoting developmental/academic growth, independence and cultural relevance.
  • Lead, schedule, and participate in coordinated service planning meetings with team members and external providers.
  • Assess the effectiveness of services and interventions delivered within the home, addressing gaps and adjusting plans as needed.
  • Maintain accurate documentation and communication regarding youth progress, service needs, and interdisciplinary coordination.
  • Participate in rotating on-call responsibilities to support program continuity and emergency response.
  • Maintain the staffing schedule for the Horizons program homes ensuring that the homes have qualified staff working in the homes at all times.
  • Provide direct supervision, coaching, performance evaluation, and operational leadership to Child Care Supervisors.
  • Ensure daily group home operations are appropriately staffed, organized, and compliant with DCFS standards and agency policies.
  • Monitor the quality and consistency of youth engagement, staff practices, safety standards, and therapeutic approaches used within the home.
  • Facilitate communication between Child Care Supervisors, clinical staff, educators and administrative leadership.
  • Offer guidance and support to ensure the timely completion of Child Care Supervisor duties such as appointment scheduling, skills-group planning, medication monitoring, and youth supervision.
  • Assist Child Care Supervisors in fulfilling their responsibilities when necessary to maintain operational stability, youth safety, or program effectiveness.
