About The Position

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities and homelessness to help them stabilize their lives and to help them live as independently as possible in the community. We are seeking a qualified Case Management Supervisor candidate interested in joining our Assertive Community Treatment (ACT) at our Claymont, Delaware location. The ACT Team Leader will provide managerial oversight to the ACT Team, manage the daily operation of the team in compliance with all state and local regulations and agency policies and procedures. The Team Leader will also provide ongoing review of participant’s placements, assessments of services and growth within the established system.

Requirements

  • Master’s degree with major in psychology, counseling, social work, sociology, nursing, rehabilitation or related field.
  • At least five (5) years of experience in the Mental Health field.
  • A minimum of 3-4 years supervisory/management experience.
  • Excellent communication skills.
  • Detailed oriented and highly organized.
  • Ability to work independently and as part of a focused team.
  • Possess a valid driver's license with an acceptable driving record.

Responsibilities

  • Develop, supervise, implement and evaluate all program systems for the ACT Team.
  • Provide leadership and overall direction to the team and staff.
  • Provide administrative supervision of ACT team personnel.
  • Schedule regular supervisory and staff meetings, case conferences, and staff training and development.
  • Coordinate, monitor and assure regulatory compliance for program service delivery.
  • Review and assess all incoming participant referrals and assign to case managers.
  • Oversee the distribution and composition of staff caseload assignments; initiate redistribution and changes as necessary.
  • Monitor and maintain an ongoing record of all participants referred directly to the ACT Team.
