As a Case Management Supervisor at Horizon, you will supervise and support TCM and PRS staff through regular coaching, training, and performance management. Manage referral management and oversight. Monitor caseloads, service delivery, productivity, and documentation quality. Ensure compliance with CCBHC, OMH, OASAS, Medicaid, and agency standards. Review documentation and conduct quality assurance activities. Assist staff with complex cases, care coordination, and barriers to engagement. Collaborate with internal teams and community partners to ensure continuity of care and successful service outcomes. Participate in program development, quality improvement initiatives, and regulatory audits. Prepare reports and maintain program performance data. Service delivery to patients as needed.

Requirements

  • Bachelor's degree in Social Work, Human Services, Psychology, Rehabilitation Counseling, or a related field required.
  • 2 years of experience in behavioral health and vocational required.
  • 2 years of supervisory or leadership experience required.
  • Knowledge of behavioral health, recovery-oriented care, and trauma-informed practices.
  • Understanding of Medicaid-funded services and community support systems.
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Proficiency in electronic health records and Microsoft Office applications.
