About The Position

Provides non-medical case management services and medical case management services under the Indiana State Department of Health, including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs. The primary goals of the HIV Case Management Program are to ensure the quality of life for individuals living with HIV. This is achieved through the thoughtful coordination of services and the empowerment of the individual.

Requirements

  • Must be able to communicate in both written and verbal form
  • Ability to read and interpret documents.
  • Ability to write routine documentation and correspondence.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Intermediate computer skills are essential in use of an electronic medical record.
  • Must be detail oriented, have ability to multi-task and possess great interpersonal skills.
  • Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
  • Must be able to pass a drug screen and background test.

  • Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel

Nice To Haves

  • A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred.
  • Candidates with case management experience of two years or more will also be considered as viable applicants.
  • Experience working in a nonprofit organization or a strong desire to work in service driven environment.

Responsibilities

  • Assess each patient on caseload at a minimum of 2x per calendar year
  • Maintain an accurate care plan and acuity list of case load.
  • Complete paperwork and case notes within 72 hours of patient contact.
  • Baseline goal of 100 MCM/NMCM (combined) services per month for each CM
  • Provide referrals to extensive network to cover patient medical and nonmedical needs
  • Work in correlation with Patient Navigators to assist with maintaining insurance coverage and part b eligibility.
