As a Social Worker within our Adult Behavioral Health Center, you will play a vital role in supporting individuals and families across diverse backgrounds and levels of need. Leveraging your expertise in crisis intervention, resource coordination, mediation, counseling, and patient empowerment, you will help patients navigate complex challenges and move toward stability, healing, and improved overall well-being. This position includes a rotating assignment supporting both inpatient and outpatient social work services within our Epilepsy program, with opportunities to provide cross-coverage for pediatric and adult populations. The role offers a dynamic clinical environment where collaboration, adaptability, and compassionate care are essential. Psychiatric social work experience is strongly preferred, as this role requires advanced clinical judgment, behavioral health assessment skills, and the ability to effectively intervene in high-acuity situations. A caregiver in this position works full-time| Mon-Fri| 8:00am—4:30pm| no holidays or weekends.

Requirements

  • Master’s Degree in Social Work from a CSWE accredited program
  • Current state licensure as a Licensed Social Worker (LSW)
  • State licensure as a Licensed Independent Social Worker (LISW) within five years of hire
  • 3 years of experience in healthcare system (to include exposure to and experience in pre acute and post-acute care preferred)
  • Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcome
  • Hospital Social Work experience
  • Knowledge of community resources
  • Manual dexterity to operate office equipment.
  • May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
  • Must have normal or correction vision
  • Must be able to clearly communicate verbally by phone.
  • Follows Standard Precautions using personal protective equipment as required.

Nice To Haves

Responsibilities

  • Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
  • Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings).
  • Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
  • Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
  • Maintain a network of resources for consultation and connect patients with community resources.
  • Supervise Social Work Interns and assist with training new caregivers.
  • Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
