About The Position

The Case Management RN Specialist functions with a high degree of autonomy, communication and interpersonal skills. Must understand the health care continuum and have the ability to solve complex problems. The Case Management RN Specialist is responsible for collecting and analyzing patient health data, identifying individualized expected outcomes, and developing and implementing patient-centered plans of care. This role prescribes and coordinates interventions to achieve desired outcomes, evaluates patient progress, and collaborates closely with the interdisciplinary healthcare team to ensure effective, high-quality care delivery.

Requirements

• Bachelor's Degree in Nursing
• Registered Nurse
• 0 years of relevant experience required
• Registered Nurse with current Ohio licensure
• Current BCLS certification
• Baccalaureate in Nursing

Nice To Haves

• 0-2 years of relevant experience preferred
• Preferred experience in primary healthcare and/or urgent care setting
• Knowledge of triage procedures
