About The Position

This role is responsible for creating a care plan across the continuum, integrating patient/family preferences and values. The Case Management RN Float Pool will monitor patient care through assessments, evaluations, and/or patient records. This role advocates for resources and removal of barriers, maintaining ongoing dialog with supervisors and other health providers to ensure effective implementation of the health plan. Additionally, the role serves as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

Requirements

Associate's of Nursing [Required]
Must have 2 years of adult care management experience in a healthcare setting
Registered Nurse (RN) [Required]

Nice To Haves

Bachelor's of Nursing [Preferred]
Basic Life Support - CPR Cert (BLS) [Preferred]
