As a Clinic Nurse (RN), you will coordinate and manage patient care plans across healthcare providers and services, assess patients’ clinical, psychosocial, and functional needs to identify appropriate interventions, and collaborate with physicians, nurses, social workers, insurance companies, and community resources. You will monitor patient progress, outcomes, and barriers to care while adjusting plans as needed, and advocate for patients by promoting quality care, safe transitions, appropriate resource utilization, and continuity of care.

Requirements

Graduate of an accredited nursing program with an active RN license in Indiana
Minimum of 2+ years of experience as an RN Case Manager
Current CPR certification from the American Heart Association
Strong assessment skills, critical thinking, and the ability to work independently
Compassionate communicator who values collaboration with students, staff, and families
