About The Position
Coordinate Program Services in the ICF-IID Services and the Medicaid Waiver Services.
Requirements
- Bachelors Degree in Human Services Area
- A minimum of 3 years of experience in a job related area.
- Ability to be insured as a vehicle driver by agency’s insurance program.
- A valid Tennessee Driver’s license.
- Good knowledge of basic English sufficient to read instructions, keep records and write reports.
- Successful completion of background and reference check.
- Successful completion of a drug screen.
- Ability to perform the essential functions and activities of the job with or without reasonable accommodations.
Responsibilities
- Supervise Medicaid Waiver and ICF-IID Case Managers and Case Manager Assistants to develop, coordinate and monitor all people supported programs related to the Support Plans
- Supervise ICF-IID Therapy Coordinator to monitor people’s physical therapy plans and equipment and to provide staff training on physical therapy plans
- Supervision Program Specialist for Medicaid Waiver and ICF-IID to coordinate behavior program plans
- Be the contact for the ICF-IID contract supports (Speech Language Pathologist, Occupational Therapist, Dietitian, PT)
- Monitor Implementation of Programs
- Be the lead representation for all surveys, develop plans of correction and ensure compliance
- Participate in Utilization Review for ICF-IID
- Coordinate and document family /conservator contact for people supported
- Assist with internal investigations for ICF-IID and DIDD investigations
- Communicate with other departments and Directors regarding people’s needs, updates or changes in people’s programs or needs and other issues or concerns that affect the programs
- Perform other duties as assigned
