About The Position

The Case Management Coordinator Per Diem gathers all relevant data related to a patient's treatment status. They objectively evaluate this data to identify barriers, clarify realistic goals, and seek potential alternatives to care. This role involves working with the patient/family, primary care physician, and multi-disciplinary teams to develop a treatment plan that enhances patient outcomes. The coordinator facilitates communication and collaboration between the patient and the health team, and advocates for patient empowerment in their own care.

Requirements

• RN required
• Two to three years hospital experience required

Nice To Haves

• BSN preferred
• Home Care and/or Utilization Review of Case Management experience desired
