  • Bachelor's Degree (BA, BS) in related field or equivalent combination of training or experience.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Nice To Haves

  • At least one-year case management experience in a human service related field.
  • Knowledge of developmental disabilities.
  • Knowledge of proactive case management practices that enhance the lives of the persons served.
  • Experience in a CARF accredited facility desired.
  • Ability to maintain a schedule that allows for visibility in assigned areas when persons served and staff are present.

Responsibilities

  • Responsible for the Individual Service Plan including: assessment administration, creation and maintenance of a comprehensive working document, training necessary staff on plan concepts, data compilation and formal review.
  • Establishes individual goals and support strategies based on input from stakeholders.
  • Gathers and reports on outcome measures related to individual client growth and customer satisfaction.
  • Initiates and coordinates Interdisciplinary Team Meetings, as necessary.
  • Serves as the central communicator to external stakeholders related to program initiatives and person served support services.
  • Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks, as required.
  • Serves as central communicator with external medical personnel.
  • Attends psychiatric appointments and oversees the behavioral health needs of residents.
  • Develops and trains Behavior Support Plans.
  • Guides staff on the use of MANDT principles and behavioral strategies.
  • Counsels persons served, assisting them to identify and resolve problems and make effective use of resources.
  • Responsible for facilitating specialized groups in program areas, as necessary.
  • Advocates for the rights of persons served while educating persons served and staff on corresponding responsibilities.
  • Assists individuals with exploring, accessing, and maintaining funding.
  • Responsible for ensuring the identified needs of the person served are in-line with financial reimbursement received.
  • Develops and maintains contact with other agency staff and local, day, and other service providers to develop specialized services for persons served.
  • Identifies gaps in the service provision and works with both internal and external resources to address identified challenges.
  • Ensures services are provided in accordance with CARF standards, funding sources and licensing agency requirements, and organizational policy and procedure.
