About The Position

The Case Assistant will work within Greenwich House’s Older Adult Network to provide individualized case management and information services that empower seniors to live independently and with dignity in their own homes. They will work closely with senior center directors and agency staff, help identify and support clients in need, and manage a caseload that includes both long-term and short-term clients. Requirements

  • Bachelor’s Degree Required
  • Ability to intermingle with the members in the daily happenings in the Older Adult Center
  • Experience working with the elderly and/or with at-risk populations
  • Fluent with benefits and entitlements
  • Must have interpersonal skills, ability to engage clients and families
  • Capacity to assess functional, cognitive, social status of clients
  • Must be able to work at other Greenwich House senior centers, off-site, and occasional home visits; will be responsible for self-transport of required company property (e.g. Laptop)
  • Ability to identify and intervene in crisis situations
  • Ability to work collaboratively with interdisciplinary team to accomplish program goals and adapt to changing environment
  • Excellent written and verbal communication skills; proficient computer skills including Microsoft Word, Excel

Responsibilities

  • Conduct client intake and comprehensive assessments to identify individual needs, strengths, and goals.
  • Provide information, case assistance, referrals, supportive counseling, and ongoing follow-up to address clients’ financial, social, health, and mental health needs.
  • Facilitate applications and renewals for public benefits, including SNAP, Medicaid, Medicare, and Access-A-Ride; assist clients in navigating complex eligibility requirements and processes.
  • Coordinate referrals and connect clients to essential services such as legal assistance, medical care, mental health services, home care, housing resources, and community-based programs.
  • Advocate on behalf of clients with public and private agencies to ensure access to appropriate services and supports.
  • Maintain timely, accurate, and confidential documentation of all client interactions, services provided, and status updates in agency records and the VIVE database, in accordance with NYC Aging and Greenwich House guidelines.
  • Monitor client progress, reassess needs as appropriate, and adjust service plans to ensure effective outcomes.
  • Conduct outreach to identify and engage isolated or at-risk seniors within the community.
  • Collaborate with senior center staff, community partners, and multidisciplinary teams to coordinate care and enhance service delivery.
  • Participate in case conferences, staff meetings, and professional development opportunities to stay informed of best practices and program updates.
  • Assist with crisis intervention and provide immediate support in urgent or high-risk situations.
  • Support program initiatives, workshops, and special projects as assigned by the direct supervisor.
