About The Position

Redwood Coast PACE with Humboldt Senior Resource Center is looking for a CarePartner to provide care for our participants in our Eureka Day Center. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. The schedule for this position is 8 am to 2:30 pm, Monday through Friday. There will be frequent staff meetings that would require an additional hour in the afternoon.

Requirements

Must have a high school diploma or GED equivalent.
Must have at least one (1) year of experience working with a frail or elderly population, preferably providing personal care in a health facility or in-home setting.
Current CPR and First Aid certification required within six (6) months of hire.
Requires clearance of a DOJ and FBI criminal history background check.
Requires pre-employment physical, and PPD skin test (TB screening).
Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to the community.
Ability to provide care for frail elderly.
Excellent interpersonal interactions, teamwork, and problem-solving skills.

Ability to adapt appropriately to change of priorities and workload.

Basic knowledge of computers and web-based applications such as timekeeping, training, and email programs.
This position requires frequent physical activity, including standing, walking, bending, reaching, and assisting participants with mobility and personal care needs.
The role involves lifting, transferring, and supporting participants, which may require exerting up to 50 pounds of force occasionally and lesser amounts frequently.
Fine motor skills and hand–eye coordination are needed for tasks such as taking vital signs, feeding, and assisting with grooming.
The position also requires visual and auditory acuity to observe participant conditions and respond to their needs, as well as the ability to maintain stamina and focus in a busy Day Center environment.
Proper body mechanics, infection control practices, and adherence to safety procedures are essential to protect both the employee and participants.

Certified Nursing Assistant, Certified Home Health Aide, or Injections Certificate preferred (additional compensation may be available for certified individuals).

Responsibilities

Assists participants who attend the Day Center with bathing, grooming, toileting, and feeding.
Accompanies participants to medical appointments in the Redwood Coast PACE Clinic as needed.
Routinely monitors vital signs such as weight, blood pressure, and pulse.
Maintains personal care supplies for the Day Center.
Sends necessary personal care supplies home with participants as needed.
Coordinates and assist with meals in the Day Center according to each participant’s Plan of Care.
Maintains a clean and sanitary environment in the Day Center at all times.
Provides nutritional supplements to participants as ordered by the Primary Care Provider and directed by the Registered Dietitian.
Assists in the monitoring of food and fluid intake according to a participant’s Plan of Care.
Assures portable meals are sent home with participants as specified by the Plan of Care.
Assists participants upon arrival in the Day Center and helps participants to prepare for departure when leaving.
Coordinates and performs tasks as directed in the task lists/calendar or under the direction of the Therapeutic Services Coordinator.
Immediately reports any observed changes in a participant’s behavior, health, or mobility to the Therapeutic Services Coordinator and other members of the Interdisciplinary Team as needed.
Demonstrates proficiency in set-up and application of therapeutic activity modalities including exercises in the Day Center.
Implements the physical therapy/occupational therapy maintenance program per the participant’s Plan of Care.
Participates on the Interdisciplinary Team as required and communicates effectively with participants, family members, caregivers, employees, and visitors.
Completes documentation in the medical record accurately and in a timely manner.
Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions per the policies and procedures.
Takes necessary measures to ensure a safety environment for oneself, co-workers, contractors, participants, visitors and others.
Monitors personal safety and attends to good body mechanics.
Ensures infection and exposure control and is prepared in case of emergency.
Assists in the monitoring of Participant safety including knowledge of fall precautions, management of wandering participants and ability to use the delayed egress system.
Consistently meets or exceeds Redwood Coast PACE targets for productivity.
Consistently meets or exceeds Redwood Coast PACE targets for customer service.
Continually seeks better ways for delivering services and communicating with participants.
Effectively collaborates with co-workers and contractors to meet Redwood Coast PACE goals and further success of the program.

Participates in the process of teaching, training, and mentoring student interns and volunteers working within the program area.
