About The Position

Under the supervision of Home Coordinators, Residential Service Coordinators, QIDPs and Residential Service Directors, the team member performs duties for the care, treatment, and safety of the individuals served with developmental/physical disabilities. This role requires reliable and prompt attendance, flexibility, and cooperation to ensure adequate facility coverage and adherence to self-scheduling guidelines. The specialist will monitor individuals served at all times, communicate information in the communication log, and verbally report to the next scheduled staff. Daily assignments include implementing Individual Service Plans, Behavior Development Programs, charting, and documentation. The role also involves initiating recreational and social activities, ensuring scheduled outings are completed, and administering medication after passing training. Maintaining a clean and safe home environment, assisting individuals with personal care, and responding to emergencies are key responsibilities. The specialist will use mechanical lifts and mobility equipment appropriately, provide safe transportation, and possess basic cooking skills or a willingness to learn, following menu plans and dietary restrictions. Maintaining positive relationships with individuals, families, guardians, and staff, monitoring health needs, documenting them, and accompanying individuals to medical appointments are also part of the role. Compliance with all Milestone, Inc. policies and procedures, including intervening in human rights issues and reporting them, is mandatory. Ordering necessary supplies is also required.

Requirements

  • Minimum age of 18.
  • Completion of Habilitation Aide Training within the first 30 days of employment.
  • High School diploma or GED.
  • Passing of the STOFHLA test.
  • Strong written and verbal communication skills.
  • Must be able to intervene physically in behavioral or health related crisis.
  • Safe driving record/approved driver.

Responsibilities

  • Monitor all individuals served at all times and communicate all information in the communication log and verbally report to the next staff scheduled.
  • Complete all daily assignments including Individual Service Plans/Personal Plan Implementation Strategies, Behavior Development Programs, and charting/documentation.
  • Initiate recreational and social activities and ensure all scheduled outings are completed.
  • Administer medication after passing all medication training.
  • Provide active, ongoing interaction to the individuals served.
  • Maintain the home in a clean, safe, healthful condition, logging repair requests correctly.
  • Assist individuals in the care of their bedroom, clothing, and personal items.
  • Assist in all emergency situations and be knowledgeable of fire extinguishers, alarms, emergency phone numbers, and disaster plans.
  • Use all mechanical lifts, belts, chairs, and other mobility equipment appropriately and clean them as necessary.
  • Provide safe transportation of individuals served using safe, defensive driving skills.
  • Possess basic cooking skills or willingness to learn, follow established menu plans, adhere to prescribed diets, develop shopping lists, purchase items within budget, and submit receipts.
  • Maintain a cordial and cooperative relationship with individuals served, their families and guardians, and other staff members, utilizing and promoting positive problem-solving skills.
  • Ensure a positive, pleasant home environment.
  • Monitor individuals’ health needs and document them; accompany individuals on medical/dental appointments as assigned.
  • Comply with all Milestone, Inc. policies and procedures.
  • Immediately intervene on behalf of the individual(s) served in compliance with Human Rights philosophy and policies, and report to the appropriate supervisor and/or agency.
  • Order necessary forms and supplies from the appropriate personnel as needed.
  • Perform other duties as needed or assigned.
