About The Position
Clarity Care is seeking a compassionate and dedicated Caregiver to join their team. As a non-profit organization, Clarity Care is committed to supporting individuals with varying abilities to lead independent and fulfilling lives. The Activity Support Provider will assist with the day-to-day operations of the Day Services Program, focusing on goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance. The ideal candidate is people-oriented, enjoys teamwork, is flexible, creative, organized, and committed to improving the quality of life for others.
Requirements
- Must possess a high school diploma/equivalent and/or related work/life experience.
- Must be at least 18 years of age.
- Must possess a valid WI driver’s license, and ability to meet agency driving standards.
- Must provide proof of auto insurance.
- Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities
- Support members in community and recreational activities.
- Assist members with physical, social, and personal needs.
- Guide members in achieving personal goals and decision-making.
- Create fun and engaging activities tailored to member interests.
- Prepare and serve meals following dietary needs and preferences.
