About The Position

Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver for a dedicated client assignment in Northeast Albuquerque, NM (87111). In this role, you will provide high-quality in-home care and delegated personal care services while supporting the client’s comfort, safety, independence, and dignity. This assignment includes assistance with intimate personal care. Consistent with the client’s privacy preferences and care needs, the client has requested a caregiver of the same gender for this specific assignment. This position is ideal for a patient, organized, and dependable caregiver who is comfortable supporting individuals with autism and mental health needs in a structured home environment.

Requirements

  • Must be at least 18 years of age
  • Ability to follow delegation instructions and care plans accurately
  • Must have a current valid driver’s license
  • Reliable transportation and valid auto insurance required
  • Must live in Albuquerque, NM or surrounding areas
  • Reliable transportation and consistent attendance required

Nice To Haves

  • Experience supporting individuals with autism and/or mental health conditions preferred
  • Bilingual English/Spanish skills are a plus
  • High school diploma or GED preferred, but not required
  • Previous caregiving experience preferred

Responsibilities

  • Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and eating
  • Follow individualized care plans and delegated care instructions provided by licensed professionals
  • Support medication reminders and delegated care tasks per care plan and agency guidelines
  • Provide mobility assistance, including transfers from bed, chair, or wheelchair
  • Maintain a clean, safe, and organized home environment through light housekeeping assistance
  • Provide structured support for a client with autism and mental health needs
  • Assist with organization, scheduling appointments, and completing errands
  • Escort or transport the client to appointments or community activities as needed
  • Observe, document, and report changes in client condition, behavior, or care needs to the clinical team
  • Maintain professionalism, confidentiality, and compassionate client-centered care at all times
