About The Position
Heritage Home Healthcare is seeking compassionate and dependable PRN caregivers in the Rio Rancho and Bernalillo, NM area to provide delegated personal care services in a home setting. In this role, you will support clients with activities of daily living, ensuring comfort, safety, and dignity. This flexible, on-call position is ideal for individuals who are reliable, observant, and passionate about providing quality in-home care.
Requirements
- Must be at least 18 years of age
- Ability to follow care plans accurately and independently
- Strong interpersonal and communication skills
- Valid driver’s license and reliable transportation

Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and eating
- Provide mobility assistance, including transfers and safe positioning
- Support medication reminders and delegated care tasks per care plan and state regulations
- Assist with light housekeeping and maintaining a safe home environment
- Provide companionship, emotional support, and encouragement
- Escort or transport clients to appointments or errands as needed
- Observe and report changes in client condition to the clinical team
- Follow all individualized care plans and instructions from licensed professionals
