About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves ensuring residents are comfortable, assisting with daily living activities, maintaining a safe and clean environment, and accurately documenting all care provided. The caregiver is expected to create and maintain a warm, positive, and calm atmosphere for the residents.

Requirements

  • Must possess, as a minimum, a high school education or its equivalent.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Proficient in Microsoft Suite products.
  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk and talk, read or hear.
  • Frequent use of all office-related equipment to include; copier/scanner/fax, telephone, and calculator.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Assist residents with activities of daily living (ALDS), including dental and mouth care, bathing, hair care, dressing, and undressing.
  • Assist residents with bowel and bladder functions, including taking them to the bathroom, offering bedpans, urinals, or portable commodes.
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents with packing personal possessions for transfers or discharges.
  • Transport residents to new rooms or receiving areas, and assist with loading/unloading to/from vehicles.
  • Make beds (occupied and unoccupied) and put extra covers on beds as requested.
  • Ensure residents who are unable to call for help are checked frequently and answer resident calls promptly.
  • Assist residents with identifying food arrangements and ensure water pitchers are clean, filled, and within reach.
  • Perform after-meal care, including removing trays and cleaning residents' hands, face, and clothing.
  • Assist residents in preparing for medical tests and physical examinations.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers.
  • Report injuries of an unknown source, including skin tears.
  • Observe disoriented and comatose residents and record/report data as instructed.
  • Provide eye and ear care, including hearing aid installation and cleaning eyeglasses/hearing aids.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs) as instructed.
  • Assist with the care of end-of-life residents and perform post-mortem care as instructed.
  • Maintain intake and output records as instructed.
  • Utilize protective and adaptive equipment as instructed.
  • Report all changes in residents' condition to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all accidents and incidents observed during the shift to the Executive Director and Health and Wellness Director.
  • Maintain confidentiality of resident protected health information and report suspected violations.
  • Report any known or suspected unauthorized attempt to access the community's information system.
  • Perform only those care tasks for which training has been received.
  • Ensure resident rooms are ready, including making beds.
  • Greet residents, escort them to rooms, and introduce them to others.
  • Inventory and mark residents' personal possessions and store clothing.
  • Create and maintain a warm, positive, and calm environment.
  • Meet with nursing personnel to identify and correct problem areas.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Immediately notify the Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Maintain confidentiality of all resident care information.
  • Report known or suspected incidents of unauthorized disclosure of information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to a supervisor.
  • Give back rubs as instructed.
  • Assist male residents with shaving and keep hair on female residents clean as instructed.
  • Position bedrest residents correctly and comfortably.
