About The Position
CareLinx by Sharecare is a nationwide caregiver registry that connects experienced caregivers with clients. Caregivers set their availability through an app, and the company matches them with clients based on schedule, preferences, and goals. The role involves providing non-clinical, in-home care to health plan members and private clients. The company emphasizes a role that respects caregiver time and expertise, with a dedicated Care Partner team to assist with finding suitable matches. Caregivers are encouraged to build consistent schedules, stay engaged through the app, and communicate proactively.
Requirements
- Minimum 6 months of caregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)
- Access to a smartphone capable of running the CareLinx app (for scheduling, documentation, and timesheets)
- Ability to communicate in English
- Reliable transportation to and from client homes
- Must pass a background check
Nice To Haves
- Bilingual caregivers strongly encouraged to apply
- Completion of program-specific training prior to accepting shifts
- Medical clearance such as TB test, physical exam, or drug screening
- Valid driver's license and MVR check if accepting driving shifts
- Additional documentation may be based on state law or client contract
Responsibilities
- Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
- Complete and document all care tasks accurately in the CareLinx mobile app during each shift
- Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
- Submit timesheets on time through the CareLinx app
- Communicate with your Care Partner regarding any case updates or client changes
- Ensure patient privacy, dignity, and well-being in a safe and secure environment
- Maintain client confidentiality at all times
- Provide transportation and errand support
- Offer hospital discharge escort services, safely accompanying members home after discharge
- Assist with telehealth navigation, helping members connect with virtual doctor visits
