About The Position

Greenwich House is seeking a dynamic, detail-oriented Caregiver Social Worker to support caregivers through core services in the Caregiver Program. Reporting to the Caregiver Program Director, this role works closely with program staff and other professionals to build community partnerships. The Caregiver Social Worker also attends staff meetings and training sessions and collaborates with organizations that serve older adults and their families. Requirements

  • Master of Social Work with good clinical skills
  • Have an awareness of the biological, psychological, and social aspects of aging and the impact of disabilities, illnesses, and aging on the family
  • Ability to arrange and negotiate service referrals
  • Skills in establishing and sustaining interpersonal relationships
  • Ability to communicate with individuals of varying socio-economic backgrounds
  • Public speaking skills
  • Proven ability to work both independently and as part of a team
  • Must be able to work occasional evenings and weekends, 1-2 per month
  • Proven ability to work both independently and as part of a team
  • Basic computer literacy – Windows, Microsoft Office
  • Must be able to travel on public transportation to conduct home visits and community presentations
  • Must be able to climb steps to conduct home visits

Nice To Haves

  • Prefer bi-lingual and cultural competency in Spanish or Haitian Creole
  • Knowledge of community resources and public benefits eligibility

Responsibilities

  • Assess caregiver stress levels and develop a mutually agreed-upon care plan to address client needs.
  • Help clients determine eligibility for benefits and entitlements, including Medicaid and SNAP, and assist with gathering and submitting application materials when needed.
  • Manage a caseload of caregiver clients by providing supportive counseling, information and referrals, advocacy, and ongoing case support.
  • Assess clients’ need and eligibility for respite and supplemental services.
  • Coordinate with community agencies to advocate for clients and help ensure they receive appropriate services, benefits, and entitlements.
  • Attend community health fairs, connect with potential caregivers, and educate attendees about available services.
  • Increase public awareness of caregiver roles, needs, and available support services.
  • Provide training and education to individual caregivers and to groups
  • Maintain accurate electronic case records and statistical reports per funder’s requirements.
  • Perform other duties as assigned.
