Requirements

  • High school graduate or G.E.D. certificate.
  • CNA or HCA license to provide home care in Washington State.
  • Ability to complete training on-site.
  • Experience as a Home Care Aide (in homes or facilities), or other applicable experience preferred.
  • Listed in good standing with the Washington DSHS Background Check System.
  • Ability to read, write, speak, and understand English.
  • Have transportation to and from the job.
  • If providing transportation for clients, must have a valid driver's license and insurance.

Nice To Haves

  • Population-specific experience (e.g., dementia, Parkinson's, hospice/end-of-life care) - training may be provided.
  • RN delegation certificate - may complete training through Journey Nursing Services.

Responsibilities

  • Perform hands-on patient care in various settings (in-home, facility, or adult family home).
  • Provide quality care that respects patient/client rights, dignity, freedom of choice, and individuality.
  • Offer individualized attention and encourage patients to maintain their highest practical physical, mental, and psychosocial well-being.
  • Understand and ensure individualized care plans are followed.
  • Report any changes in patient condition or needs to the nurse supervisor.
  • Assist with Activities of Daily Living (ADLs) including grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, and communication.
  • Promote skin integrity to prevent pressure ulcers and other skin damage by changing incontinent residents, repositioning immobile residents, and applying moisturizers.
  • Treat patient/client personal property with respect.
  • Perform delegated tasks assigned by the nurse, including taking vital signs, weighing residents, and applying creams/ointments.
  • Lift up to 50 lbs using proper body mechanics or lifting devices.
  • Understand and follow safety and infection control procedures, including hand washing and proper disposal of materials.
  • Demonstrate versatility, flexibility, and enthusiasm in a constantly changing environment with a client-first attitude.
  • Communicate and interact effectively and tactfully with residents, visitors, families, peers, and supervisors.
  • Foster a culture of excellence and provide reliable, safe, high-quality patient care.
  • Reflect the Journey Culture when in the community.
  • Perform other job-related duties as needed.
