Requirements

  • High school graduate or G.E.D certificate
  • CNA or HCA license to provide home care in Washington State.
  • Ability to read, write, speak, and understand English as needed for the job
  • Have transportation to and from job.
  • If providing transportation clients, you must have a valid driver's license and insurance.
  • Be listed in good standing with the Washington DSHS Background Check System

Nice To Haves

  • Experience as a Home Care Aide (in homes or facilities), or other applicable experience preferred.
  • Population-specific experience a plus, (training may be provided) for conditions such as: dementia, Parkinson's, hospice/ end of life care.
  • RN delegation certificate a plus may complete training through journey Nursing Services.

Responsibilities

  • Perform hands-on patient care at the place of assignment: In-home, Facility, or Adult Family Home.
  • Provide quality care to patients/clients in an environment which promotes patient/client rights, dignity, freedom of choice, and individuality.
  • Provide individualized attention and encourage residents to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Understand and ensure that individualized care plans are followed.
  • Report any change of condition or observation of patient need to nurse supervisor.
  • Assist with ADLs: including assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals’ care requirements as directed.
  • Promote skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin.
  • Treat each patient client's personal property with respect.
  • Perform various delegated tasks assigned by the nurse including vital signs, residents’ weights, and applying creams/ointments.
  • Understand and be able to follow safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
  • Communicate and interact effectively and tactfully with the resident, visitors, families, peers, and supervisors.
  • Foster a culture of excellence and provide exceptionally reliable, safe, high-quality patient care.
  • Perform other job-related duties as needed.
