About The Position

In partnership with the health services team, the Memory Care Caregiver is responsible for assisting residents in Memory Care in all areas of care, engagement, and dining as assigned by the Enliven Director, or designee. This role adheres to and conveys the SRG philosophy of person-centered care and Genuine Hospitality, maintaining a professional appearance and approach while on duty. The caregiver reports to work on time, receives reports from the previous shift, and completes incident reports when needed. They are responsible for documenting care given via CareStream, providing reports to the oncoming shift, and answering resident call lights and alarms promptly. The role involves assisting residents with activities of daily living, meals, housekeeping, and laundry services, as well as setting up and cleaning dining and kitchen areas. The caregiver will conduct engagement activities as assigned and maintain a clean, safe environment for residents, staff, and visitors. They will observe for changes in resident status, respond to emergencies, and follow infection control and emergency procedures. Confidentiality of resident and family information is paramount, and attendance at training and team meetings is required. Other duties may be assigned.

Requirements

CNA or experience as a caregiver
High school diploma or GED
First Aid / CPR Certification required

Experience with Yardi, eMAR, Schedule Pop, ADP, or other relevant platforms.

Responsibilities

Adheres to and conveys the SRG philosophy of person-centered care and Genuine Hospitality.
Maintains a professional appearance, manner, and approach while on duty.
Wears name tag, uniform and adheres to SRG cell phone policy.
Reports to work on time and as scheduled per community policies and procedures.
Receives report from the previous shift, reads Care Staff Assignment and End of Shift report for information and changes before providing resident care.
Completes incident reports when needed.
Sign out and carry equipment such as radio, tablet, and keys at onset of shift.
Documents care given via CareStream; adhering to policies and procedures.
Provides report to oncoming shift and documents on the End of Shift Report and CareStream before going off shift.
Answers resident call lights, door, and resident alarms promptly.
Provides resident assistance with activities of daily living, encouraging, and promoting residents’ self-care to the extent possible as needed or assigned.
Assist residents with meals, housekeeping, and laundry services as needed, requested or assigned.
Sets up dining room, serves food, and cleans up dining and kitchen area.
Conducts engagement as assigned, encourages and assists residents in individual and group program activities.
Maintains a clean, neat, comfortable, safe environment for residents, staff, and visitors.
Provides high quality resident care and recommends ways to improve quality.
Observes for changes in resident physical or mental status and communicates to the Enliven Director, LVN/LPN, and/or manager on duty.
Responds promptly to residents, staff, or visitor emergencies, follows policies and procedures.
Demonstrates knowledge of and follows policies and procedures for infection control.
Familiar with fire, missing person, and other emergency procedures; follows community policy and procedures detailed in the fire and disaster plans.
Maintain appropriate levels of confidentiality with resident and family information.
Attend and participate in training and team meetings as assigned.
Other duties as assigned.
