This role provides support for individuals and/or groups of persons with disabilities in the community as part of waiver services. Direct Support Professionals assist individuals in identifying and communicating their requests and needs for services and supports, and implement services to address their goals and desired outcomes as identified through each person’s Centered Plan. The position involves understanding individuals in the context of their personal history, social and family networks, and their hopes and dreams for the future. This is an entry-level role where you will be a key member of a care team, helping people identify and achieve their goals and live their best lives, acting as an advocate, cheerleader, teacher, shopping partner, role model, and coach, supporting people in both their home and community.
