Requirements

  • Proven experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience.
  • Strong knowledge of patient observation techniques, vital signs measurement, medication administration, and medical terminology.
  • Skilled in assisting with ADLs and providing dementia or Alzheimer's care with patience and empathy.
  • Familiarity with EMR/EHR systems for accurate documentation of patient records and medical history.
  • Ability to operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely.
  • Excellent communication skills for working effectively with patients who have developmental disabilities or cognitive impairments.
  • CPR certification preferred; additional certifications like hospice care training or working with individuals with disabilities are a plus.

Responsibilities

  • Deliver compassionate assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts.
  • Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; accurately document medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
  • Assist with medication administration following proper protocols to ensure safety and compliance with HIPAA regulations.
  • Support patients with specialized care needs such as catheter care, dementia or Alzheimer's care, hospice support, and memory care routines.
  • Conduct patient observation to identify changes in condition or behavior; communicate findings promptly to supervising healthcare professionals.
  • Prepare nutritious meals and assist with meal planning while maintaining cleanliness through light housekeeping tasks like laundry and cleaning to ensure a safe environment.
  • Maintain thorough medical documentation and ensure compliance with privacy standards; participate in ongoing training related to medical terminology and caregiving best practices.
