About The Position

Join Our Team as a Caregiver Case Manager! Are you passionate about supporting the elderly population, and making a real difference in the nonprofit sector? EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a full time Caregiver Case Manager to join the National Family Caregiver Support program. Salary: $28 to 32 per hour Schedule: Monday-Friday, 9AM to 5PM with occasional evening hours for home visits or outreach Location: 99 Quentin Roosevelt Blvd in Garden City What drives the core purpose of this job: The Caregiver Case Manager provides intake, assessment, care planning, and case management services to caregivers and care recipients. The position supports access to services and ensures coordination of care to maintain individuals safely in the community under the clinical oversight of the Program Director.

Requirements

  • Bachelor’s Degree in Social Work required; Master’s Degree in Social Work preferred.
  • Experience working with older adults, caregivers, or in human services.
  • Experience conducting home visits and service coordination preferred.
  • Strong organizational, documentation, and case management skills.
  • Ability to work with diverse populations and complex family dynamics.
  • Proficiency in Microsoft Office and database systems.
  • Valid NY State driver’s license and access to a vehicle required for traveling to North East and North Central Nassau, Long Island.

Responsibilities

  • Conduct caregiver and care recipient assessments, including home visits as required.
  • Develop and implement individualized service plans based on identified caregiver needs.
  • Provide ongoing case management, including follow up and reassessment.
  • Provide information and assistance, including referrals to community based services.
  • Coordinate access to respite and supplemental services in collaboration with program staff.
  • Monitor client progress and update service plans as needed in consultation with the Program Director as appropriate.
  • Maintain accurate and timely case records in accordance with program and funder requirements.
  • Assist caregivers in navigating benefits, entitlements, and service systems.
  • Participate in case reviews and team meetings.
  • Support program outreach and enrollment efforts targeting caregivers with identified risk factors.
  • Prepare required documentation and contribute to program reporting.
  • Ensure compliance with all EAC Network policies and applicable regulatory requirements.
