About The Position

Provides leadership and direction to Long-term Services staff within programs specifically designed to meet the needs of clients with cognitive, physical, and/or behavioral challenges. Responsible for knowledge of and implementing Person Center Plans (PCPs) and behavior plans of each client.

Requirements

  • Associate degree or equivalent from two-year college or technical school and two years related experience and/or training or equivalent combination of education and/or experience.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole number, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Microsoft Outlook, Word and Excel; Internet Explorer
  • Medication Administration, CPR/AED and First Aid, Non-Violent Physical Crisis Intervention to be obtained within one year of employment.

Responsibilities

  • Provide supervision and leadership to assigned employees to include interviewing and hiring process, orientation, counseling and/or discipline, required training, performance evaluation and enforcement of safety standards.
  • Electronically review and approve staff time sheets (excluding leave without pay).
  • Inform Associate Director of all personnel issues.
  • Conduct regular Team meetings and provide reports regarding team issues.
  • Actively participate as a member of the Long-term Services Leadership (LTS) team, to include working cooperatively, communicating clearly, and providing leadership for the agency staff and clients.
  • Participate in the development of the agency strategic and operation plans and facilitate development of divisional goals and objectives. Provide leadership for accomplishment of these plans.
  • Work cooperatively with all staff, service providers, families, service agencies and other stakeholders.
  • Assist with budget preparation and maintain respective programs within the allocated budget.
  • Responsible for the reporting and monitoring of health and safety issues regarding the clients, staff, physical building and grounds.
  • Responsible for ordering program supplies and maintaining inventories.
  • Follow all safety policies and procedures
  • Ensure the development, planning, coordination, and implementation of activities for clients to participate in based on client needs (on site and in the community).
  • Interact with clients appropriately and provide a good role model.
  • Provide input, monitor and implement client Person Center Plans (PCPs) and behavior plans.
  • Maintain and document daily progress towards client goals and department outcomes.
  • Review and initial client timecards.
  • Assist Service Coordinator in promoting behavioral changes; develop, implement, and monitor behavior intervention strategies as needed.
  • Schedule, monitor, and assist (as appropriate) in the provision of personal care needs (i.e., grooming, transfers, eating, personal cares, repositioning, etc.), and activities of daily living, community participation of clients.
  • Maintain financial records related to the money in the client’s recreation/program fund.
  • Keep Associate Director informed of all pertinent client issues.
  • Administer medications as needed.
  • Directly supervises employees in Long-term Services programs. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
