About The Position
The Care Transitions Nurse provides clinical assessment and care coordination to support safe and effective transitions between care settings. The role evaluates clinical needs, collaborates with providers, and ensures care plans align with clinical standards to support appropriate care and outcomes. The position is remote within Trillium Health Resources’ Southern Region, with travel within the catchment area as required.
Requirements
- Associate's degree and one year of experience as a Registered Nurse (RN).
- Fully licensed RN in the state of North Carolina.
- Valid driver's license.
- Must reside within Trillium’s Southern Region: Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, or Scotland County.
- Must be able to travel within the catchment area as required.
- Must be legally authorized to work in the United States without requiring employment sponsorship now or in the future.

Responsibilities
- Provide clinical assessment and oversight to support safe transitions between care settings.
- Evaluate medical needs and determine appropriate levels of care during transitions.
- Collaborate with providers and care teams to ensure clinical alignment of services and treatment plans.
- Monitor clinical status during transitions and identify changes requiring intervention.
- Support coordination of medical services and facilitate referrals to appropriate healthcare providers.
