About The Position
We are seeking a dedicated and detail-oriented Care Transitions Assistant to assist the Director of Care Transitions in managing home health services, hospice referrals, and guest notices. Requirements
- Excellent organizational and time management skills
- Strong attention to detail and accuracy

- Ability to handle sensitive information with confidentiality
- Proficiency in office software and filing systems

- Prior experience in healthcare administration or related field preferred
Responsibilities
- Assist the Director of Care Transitions with coordinating home health services and hospice referrals
- Issue notices and communicate effectively with guests and their families
- Maintain accurate and organized filing systems for all documentation
- Ensure attention to detail in all tasks to support seamless care transitions
- Support administrative tasks related to care coordination and documentation
- Collaborate with healthcare providers and team members to facilitate smooth service delivery
