About The Position

The Care Transitions Navigator coordinates activities that promote quality outcomes, patient throughput, and discharge planning while balancing optimal care with appropriate resource utilization. The role identifies potential barriers to patient throughput and quality outcomes to minimize delays in discharge plans. The position is PRN, with hours of 0800–1630, Monday through Friday as needed, and some weekends.

Requirements

• Bachelor’s degree in Social Work; a Master’s degree in Social Work or a Registered Nurse with a BSN is preferred.
• One year of related work experience.
• Ability to prioritize multiple tasks in a fast-paced work environment.
• Ability to periodically flex the work schedule as indicated by client or hospital needs.
• Ability to develop and maintain good working relationships with all levels of staff.
• Ability to communicate articulately, verbally and in writing, and demonstrate empathy, flexibility, and objectiveness.
• Ability to maintain a professional approach to handling confidential information.

Nice To Haves

• Hospital case management experience preferred.
• LMSW or LBSW, or RN licensed by the Texas Board of Examiners, preferred.
• CCM or ACM preferred.
• Master’s degree in Social Work or RN with BSN preferred.
