Requirements

Master's Degree in Social Work is required
LMSW - Licensed Master Social Worker is required upon hire or a LCSW - Licensed Clinical Social Worker is required upon hire
CPR - Cardiopulmonary Resuscitation (BLS) is required upon hire
Knowledge of Microsoft Outlook and Office (Word, Excel)
Customer service skills
Ability to engage in complex clinical decision-making
Strong oral and written communication skills
Strong commitment to interdisciplinary collaboration
Critical thinking, analysis and conflict resolution skills
Psychosocial and crisis intervention skills
Ability to prioritize and meet deadlines

Nice To Haves

3 years of Social Work experience in a hospital/medical environment is highly preferred
Case Management / Social Work experience in a hospital setting is highly preferred
Case Manager / Social Work experience in a post-acute care facility helpful
1 year of discharge planning/care management in a hospital setting is preferred
ACM - Accredited Case Manager OR CCM - Certified Case Manager is preferred upon hire
Working knowledge of medical necessity criteria preferred
