About The Position

Join Community Health Network, a community-focused organization with over 60 years of experience providing exceptional care. We are seeking a Care Transition Liaison on the Innovative Healthcare Collaborative of Indiana (IHCI) Team, a joint venture between Community Health Network and Deaconess Health System. This hybrid position involves working at Deaconess Gateway Hospital and remotely. The Care Transition Liaison will make a profound impact on the lives of people living with complex and/or chronic conditions. The role ensures that value-based patients discharged from the hospital receive disease-specific education, are connected to a primary care physician, and receive assistance with discharge planning. You will partner with IHCI and hospital teams to advocate for patients and create seamless care transitions for a safe and successful return home. This role involves close collaboration with inpatient case management teams to coordinate consultations, referrals, and community resources.

Requirements

  • 2 year / associate degree in nursing (required)
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) (required)
  • 5+ years of nursing experience required, preferably in an inpatient setting.

Nice To Haves

  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) or equivalent certification.

Responsibilities

  • Ensuring value-based patients discharged from the hospital have been provided with disease specific education
  • Connecting patients to a primary care physician
  • Assisting patients with discharge planning
  • Partnering with other IHCI and hospital teams to serve as patient advocates
  • Creating seamless care transitions for patients being discharged from the hospital
  • Working closely with inpatient case management teams to coordinate consultations, referrals and community resources to support the patient upon discharge from area hospitals
