About The Position

Sentara Health is seeking to hire a qualified individual to join our team as a Care Transition Coordinator. This is a float position that will cover locations in the Hampton Roads area where the Care Transition Coordinator is out of the office for reasons such as PTO or call out. The Care Transition Coordinator is responsible for the identification and assessment of home care and hospice needs of the patient population and coordination of these services to ensure a smooth transition to the next site of care.

Requirements

• Licensed RN, LPN, RT or BSW required
• Proof of education as LPN, RN, RT or BSW required
• 1 year of home care, discharge planning or case management experience required

Responsibilities

• Identification and assessment of home care and hospice needs of patient population
• Coordination of home care and hospice services to ensure a smooth transition to the next site of care
