About The Position

Helen Hunt Health Center is seeking a mission-driven Nurse Practitioner or Physician Associate. In this role, you will work with PCHC established patients and with new patients in a same-day acute care setting. Same day care is defined as care that is urgent and/or problem focused which cannot wait until the patient can see their primary care provider. Your team will include Helen Hunt Health Center’s outpatient Primary Care providers, other Care Team providers, Primary Care RNs, MAs, and other disciplines embedded in the practice but also include providers throughout the PCHC organization. The Care Plus provider will support increasing access to all patients by caring for new patients, seeking same-day care, and aid in seamless transitions of care during provider or service changes. · Providers will not have a panel of patients but will focus on acute outpatient primary care same-day visits. · Full -time 40 hours per week (4) 10 hr days. · Providers will rotate between Acute Care Setting (new and existing patient 2 days) and Care Team (established patients only) in the same-day service setting at Helen Hunt Health Center as needed. Physician Associate: On June 16, 2025, Maine enacted LD 1166, "An Act to Change the Professional Title and Identification of Physician Assistants to Physician Associates." This law changes the title only. The law, however, does not change scope of practice, licensure process, supervision, or billing. Requirements

  • Graduation from an accredited Nurse Practitioner and/or Physician Assistant /Physician Associate program.
  • Current license to practice medicine as a Nurse Practitioner and/or Certified Physician Assistant in the State of Maine.
  • Current DEA license.
  • Will have up-to-date training and certification in Basic Life Support issued by American Heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
  • Minimum of one year of clinical experience is preferred.

Responsibilities

  • Collaborate with primary care providers (PCP) and nursing staff to support PCHC patients and new patients.
  • Ensure access to same-day care for everyone, addressing immediate needs.
  • Assist in the management of patients during transitions, such as when a PCP leaves the organization or during provider absences.
  • Provide urgent/same day care for unpaneled patients until they are assigned a permanent provider.
  • Support desktop management for PCPs to facilitate efficient care delivery.
  • Uphold a patient-centered approach to care, prioritizing quality, and access.
